Daycare Incident Report

Young children play actively and accidents are unavoidable. Childcare center providers can use this childcare incident template to report any incident that happens at your center, including a description of the event and the treatment given.

Health and safety is top-of-mind in successful daycare centers, and childcare providers must do their best to keep the children in their care safe and report any injuries to the parent/guardian of the child. Accidents happen, and it’s important to have a process in place when there’s a daycare injury, including a serious injury like a broken bone that needs immediate medical attention, or a less concerning injury that may need first aid medical treatment but might not be as dire. An accident report or incident report form is an essential part of the communication from caregivers to teh child’s parent.
